Nav-CARE

What is Nav-CARE?

Nav-Care is a program in our region, sponsored by the South West District Palliative Care Board.  We offer support to individuals who may be going through a period of loneliness, has a chronic condition or is in declining health.  Our clients do not need to be palliative.

Nav-CARE support is offered at no cost to our clients.  The support we offer is of a social nature, focusing on improving quality of life.  We also assist with navigating available resources to allow individuals to live in their homes longer.

We are excited to meet you and match you with one of our specially trained volunteers.

How Can I Access Nav-CARE?

Clients are able to self refer or be referred by a friend, family member, or other service provider.  Simply reach out to our Volunteer Co-ordinator and you will be guided through the process.

What do Nav-CARE Volunteers do?

  • Help locate local services and resources to help manage declining health
  • Help you get to where you need to go
  • Assist you in re-engaging in hobbies and other interests
  • Talk you through important decisions and future plans
  • Help relieve feelings of loneliness, isolation, and anxiety
  • Offer one-on-one relationships tailored to your needs
